Terms & Conditions

Please read this Agreement carefully before accessing or using the Service, because it sets forth the important terms you will need to know about CoverWell’s closed-loop Cosmedicard points and warranty program. In this Agreement, the terms “you” and “yours” refer to the person enrolling in or using the Service.

Although accessible by others, the Service is intended for access and use only by U.S. customers.

CoverWell is not a medical practice and should not replace consultation with your primary care provider.

THIS AGREEMENT IS SUBJECT TO CHANGE AS PROVIDED HEREIN. If we need to change the terms of this Agreement in the future, we will post the updated Terms & Conditions through our Platform. By continuing to access or use the Service after we post such changes, you agree to the terms of this Agreement, as modified.

Program Overview

CosmediCard is a closed loop digital points program operated by CoverWell and delivered as a business services platform. Customers purchase CosmediCard points for use only within the CoverWell network of participating cosmetic, aesthetic, wellness, and health related providers.

CosmediCard is not an open loop gift card, stored value card, prepaid card, or cash equivalent. CoverWell does not sell, arrange, bundle, or prepay medical services, products, procedures, subscriptions, or insurance. No physical card is issued.

By purchasing or using CosmediCard points, you agree to these Terms and Conditions.

 

Acceptance of Terms and Conditions

Your access to and use of the Service is subject to this Agreement, as well as all applicable laws and regulations. If you do not accept and agree to be bound by this Agreement in its entirety, you are strictly prohibited from visiting, accessing, registering with or using the Service or any information or Content provided through the Service (other than as necessary to review this Agreement).

We may revise or remove any part of the Service at our sole discretion at any time, without prior notice to you. Any changes to this Agreement are effective upon posting to the Platform. Unless otherwise indicated, any new Content added to the Service is also subject to this Agreement upon posting. If you disagree with this Agreement or any term herein, your sole remedy is to discontinue your use of the Service.

Your Relationship with Us

CoverWell partners with manufacturers, distributors, merchants, and healthcare providers (“Providers”) to offer customers limited contractual warranty coverage (the “Coverage”). By accepting this Agreement, you acknowledge and agree that:

  • CoverWell is not a healthcare provider, insurer, or guarantor of medical outcomes.

  • CoverWell acts as program administrator, facilitating access to warranties, warranty point protection, and bundled plan benefits.

  • Services, treatments, or products you receive from Cosmedicard network participants are subject to their own practices, policies, and terms; Merchants and Providers are independent third parties.

  • CoverWell does not control or influence the medical decisions or care delivered by Cosmedicard network participants, and makes no representation regarding their performance or outcomes.

By enrolling, you create a direct customer relationship with CoverWell to access the Cosmedicard Program.. You are not establishing a medical–patient relationship with CoverWell.

Nature of the Transaction

At the time of purchase, the customer is purchasing:

  • Closed loop CosmediCard points

  • A built in, limited contractual warranty related solely to the functionality and usability of those points

  • Key characteristics of the transaction:

    • Points are non cash equivalent

    • Points are non transferable and non redeemable for cash

    • Points may only be redeemed with participating Cosmedicard network participants

    • The purchase is a one time transaction

    • No recurring billing or subscription obligation exists

    • No medical services are sold, bundled, or prepaid through CoverWell

This transaction represents participation in a closed loop points program with an accompanying contractual service feature.

Notice Regarding Your Financial Responsibility

CoverWell’s closed-loop Cosmedicard points and warranty program coverage operates outside of federal, state, or private insurance programs. This means:

  1. Coverage is offered on a direct-payment or self-pay basis.

  2. Neither you nor CoverWell will submit claims for reimbursement to Medicare, Medicaid, or private insurance.

  3. You are entirely responsible for costs not included in your Coverage plan.

By enrolling, you explicitly choose to obtain services and products on a self-pay basis and accept full financial responsibility for your participation in Cosmedicard’s warranty protection program.

Contractual Warranty Disclosure

Each CosmediCard includes a limited contractual warranty. This warranty is:

  • Contractual in nature

  • Limited in scope

  • Tied exclusively to the validity, functionality, and usability of the CosmediCard points

The warranty does not:

  • Cover medical services, procedures, or outcomes

  • Guarantee distributor, manufacturer, merchant or provider performance

  • Function as insurance

  • Pool risk or involve actuarial pricing

The warranty is not a regulated insurance product or financial instrument. Full warranty terms are disclosed prior to purchase and governed by the customer agreement.

Coverage is limited to products, services, or treatments specifically included in your selected plan. Exclusions include unrelated medical conditions, services or providers outside of the program, and benefits beyond your plan’s defined scope. All benefits are subject to limits, exclusions, provider compliance, and plan rules. Coverage is nontransferable and cannot be combined with other protection programs or insurance benefits.

Contractual Warranty Disclosure

CoverWell does not provide, arrange, recommend, or control medical care. CoverWell network participants  are independent third party businesses. Network participants independently set pricing, services, and eligibility. Any clinical decisions occur solely between the customer and Cosmedicard’s network participant. CoverWell is not involved in diagnosis, treatment, or outcomes. Any dispute related to medical services must be resolved directly with Cosmedicard’s network participant.

Eligibility & Enrollment

  • You must be at least 18 years old (or the age of majority where you reside) to enroll.

  • You must furnish accurate, complete, and truthful information at enrollment and maintain that accuracy over time.

  • CoverWell reserves the right to verify eligibility and to suspend or terminate enrollment for failure to comply or fraud.

Customer Responsibilities

Customers agree to:

  • Provide truthful, current information throughout your participation.

  • Comply with CoverWell’s network participant directions and care protocols.

  • Make timely payments under your plan.

  • Use services only through participating CoverWell network participants and approved channels.

Distributor, Manufacturer, Merchant and Provider Responsibilities

Cosmedicard’s network and program participants agree to:

  • Deliver care and services consistent with program requirements and standards.

  • Disclose pricing, coverage inclusions, and exclusions transparently.

  • Safeguard customer data in compliance with applicable privacy laws.

  • Communicate clearly about what is and is not covered.

Payment Terms & Billing

  • Your plan will specify payment terms, due dates, and accepted methods.

  • Payments will be processed via secure systems.

  • Failure to pay may result in suspension or termination of your coverage.

Cancellations & Refunds

  • You may cancel your participation by contacting CoverWell in writing or via our support interface.

  • Refund eligibility (if any) is dictated by the specific plan terms.

  • CoverWell may terminate your coverage for non-payment, fraud, or misuse.

Redemption and Use of Points

CosmediCard points may only be redeemed with participating program participants within the CoverWell network and only for eligible goods or services offered by that business. Points cannot be used outside approved merchant categories or for unlawful transactions.

CosmediCard points redeem at a one to one dollar equivalent value within the network. Unused points remain available for future use with the issuing Cosmedicard’s network participant:, subject to applicable state law. If a purchase exceeds the available point balance, the remaining amount must be paid directly to the provider using another payment method.

CosmediCard balances are subject to applicable state gift card and promotional credit laws. Requirements are handled on a case by case basis based on customer location.

Lost, Stolen, or Misused Points 

CosmediCard points are delivered digitally upon purchase.

  • CoverWell is not responsible for lost, stolen, or compromised access.

  • Points will not be replaced or refunded except where required by law.

  • CoverWell may suspend or cancel points in cases of suspected fraud or misuse.

Transfer, Resale, and Promotional Restrictions
CosmediCard points:

  • May not be resold or transferred for value.

  • May not be exchanged for cash.

  • May not be used for promotions, sweepstakes, or marketing without written consent.

Refund & Return Policy

CosmediCard points are non refundable and non reversible, except where required by law. Once issued, points cannot be exchanged for cash or reissued for any reason.

  • Non-refundable: Cosmedicards and other purchases are non-refundable and cannot be returned.

  • No cash redemption: Cosmedicards cannot be redeemed for cash.

  • Cosmedicard Network participant refunds and returns: Refunds for services purchased with a Cosmedicard are handled directly by the manufacturer, distributor, or merchant, not CoverWell. If a network participant authorizes a refund for goods or services purchased using CosmediCard points, that refund is handled solely by the network participant according to their policies. CoverWell is not responsible for merchant refunds, cancellations, or disputes.

Cancellation Policy

CoverWell may cancel the Agreement for nonpayment, fraud, misrepresentation, or by mutual agreement. Written notice will be sent at least 5 days before cancellation. A prorated refund will apply. Neither you nor CoverWell are obligated to renew beyond the current plan period.

Privacy & Data Protection

CoverWell collects only the data necessary to administer its closed-loop Cosmedicard points and warranty program. Customer information is used and stored in accordance with our Privacy Policy. We comply with all applicable privacy laws, and do not sell or disclose personal data to unauthorized parties.

Limitation of Liability

CoverWell is not insurance, does not provide medical care, and does not guarantee clinical outcomes. To the fullest extent permitted by law, CoverWell and its affiliates disclaim liability for indirect, incidental, or consequential damages arising from your use of the Service.

Dispute Resolution

If a dispute arises, you agree first to contact CoverWell to try to resolve it informally. If unresolved, disputes will be governed by binding arbitration under applicable state law. You waive the right to participate in class actions.

Modifications to Terms

We reserve the right to update or change these Terms at any time. Modifications become effective immediately upon posting. Continued use constitutes acceptance of the revised Terms.

COSMEDICARD FAQs.

  • Once a card has shipped, the card is not refundable. If the intended recipient does not receive their Gift Card, please contact support at 561-537-3320 or by going to our Contact Us page and submitting a request.

  • Our Cosmedicards do not have any expiration dates or fees. If an expiration date is indicated on E-Gift Cards and/or Physical Gift Cards, and should you keep the card until that date with unused funds, simply reach out to CoverWell for another card. For further questions, please refer to the specific Terms and Conditions found on the product page prior to purchasing your Gift Card.

  • You can check the status of your order by using our Contact Us page in the footer section of the site or give us a call and we will help you with your inquiries.

  • Physical Cosmedicards can only be shipping to the shipping address provided at the time of purchase. They arrive in a single envelope or box (depending on order size) containing all of the cards that were ordered, along with a packaging list detailing the contents.

  • Once payment is cleared (usually within minutes), it will take up to 3 business days to fulfill your order. Delivery timing is dependent upon the shipping method you select at checkout.

  • We offer delivery options ranging from ground shipping to next day shipping. Freight charges will be quoted based on the size of the order and the delivery method selected.

  • Cosmedicards hold a live value once purchased and can be redeemed by the participating location of your choice. Please always present your CoverWell Cosmedicard upon arrival to your appointment. Cosmedicards are to be redeemed like a credit card.
