CoverWell (“CoverWell,” “we,” or “us”) owns and operates the websites located at www.coverwell.co and may now or in the future own and/or operate a CoverWell mobile application (collectively, the “Platform”). Your access and use of the Platform, any part thereof, or anything associated therewith, including its content (“Content”), any products or services provided through the Platform or otherwise by CoverWell, and any affiliated website, software, or application owned or operated by CoverWell (collectively, including the Platform and the Content, the “Service”) are governed by these Terms & Conditions (“Terms and Conditions” or “Agreement”).
Although accessible by others, the Service is intended for access and use only by U.S. customers.
Please read this Agreement carefully before accessing or using the Service, because it sets forth the important terms you will need to know about CoverWell’s warranty patient protection program. In this Agreement, the terms “you” and “yours” refer to the person enrolling in or using the Service.

CoverWell is not a medical practice and should not replace consultation with your primary care provider.
THIS AGREEMENT IS SUBJECT TO CHANGE AS PROVIDED HEREIN. If we need to change the terms of this Agreement in the future, we will post the updated Terms & Conditions through our Platform and update the “Last Updated” date. By continuing to access or use the Service after we post such changes, you agree to the terms of this Agreement, as modified.
Acceptance of Terms and Conditions
Your access to and use of the Service is subject to this Agreement, as well as all applicable laws and regulations. If you do not accept and agree to be bound by this Agreement in its entirety, you are strictly prohibited from visiting, accessing, registering with or using the Service or any information or Content provided through the Service (other than as necessary to review this Agreement).
We may revise or remove any part of the Service at our sole discretion at any time, without prior notice to you. Any changes to this Agreement are effective upon posting to the Platform. Unless otherwise indicated, any new Content added to the Service is also subject to this Agreement upon posting. If you disagree with this Agreement or any term herein, your sole remedy is to discontinue your use of the Service.
Your Relationship with Us
CoverWell partners with manufacturers, distributors, and healthcare providers (“Providers”) to offer patients financial protection and bundled care plans (the “Coverage”). By accepting this Agreement, you acknowledge and agree that:

CoverWell is not a healthcare provider, insurer, or guarantor of medical outcomes.

CoverWell acts as program administrator, facilitating access to warranties, warranty patient protection, and bundled plan benefits.

Services, treatments, or products you receive from Providers are subject to their own practices, policies, and terms; Providers are independent third parties.

CoverWell does not control or influence the medical decisions or care delivered by Providers, and makes no representation regarding their performance or outcomes.
By enrolling, you create a direct customer relationship with CoverWell to access the Platform and Coverage plans. You are not establishing a doctor–patient relationship with CoverWell.
Notice Regarding Your Financial Responsibility
CoverWell’s warranty patient protection coverage operates outside of federal, state, or private insurance programs. This means:

Coverage is offered on a direct-payment or self-pay basis.

Neither you nor CoverWell will submit claims for reimbursement to Medicare, Medicaid, or private insurance.

You are entirely responsible for costs not included in your Coverage plan.
By enrolling, you explicitly choose to obtain services and products on a self-pay basis and accept full financial responsibility for your participation in the warranty patient protection program.
Coverage Details & Limitations

Coverage is limited to products, services, or treatments specifically included in your selected plan.

Exclusions include unrelated medical conditions, services or providers outside of the program, and benefits beyond your plan’s defined scope.

All benefits are subject to limits, exclusions, provider compliance, and plan rules.

Coverage is nontransferable and cannot be combined with other protection programs or insurance benefits.
Eligibility & Enrollment

You must be at least 18 years old (or the age of majority where you reside) to enroll.

You must furnish accurate, complete, and truthful information at enrollment and maintain that accuracy over time.

CoverWell reserves the right to verify eligibility and to suspend or terminate enrollment for failure to comply or fraud.
Patient Responsibilities
Patients agree to:

Provide truthful, current information throughout your participation.

Comply with Provider directions and care protocols.

Make timely payments under your plan.
-
Use services only through participating Providers and approved channels.
Provider Responsibilities
Providers in the program agree to:

Deliver care and services consistent with program requirements and standards.

Disclose pricing, coverage inclusions, and exclusions transparently.

Safeguard patient data in compliance with applicable privacy laws.

Communicate clearly about what is and is not covered.
Payment Terms & Billing

Your plan will specify payment terms, due dates, and accepted methods.

Payments will be processed via secure systems.

Failure to pay may result in suspension or termination of your coverage.
Cancellations & Refunds

You may cancel your participation by contacting CoverWell in writing or via our support interface.

Refund eligibility (if any) is dictated by the specific plan terms.

CoverWell may terminate your coverage for non-payment, fraud, or misuse.
Privacy & Data Protection
CoverWell collects only the data necessary to administer its warranty patient protection program. Patient information is used and stored in accordance with our Privacy Policy. We comply with all applicable privacy laws, and do not sell or disclose personal data to unauthorized parties.
Limitation of Liability
CoverWell is not insurance, does not provide medical care, and does not guarantee clinical outcomes. To the fullest extent permitted by law, CoverWell and its affiliates disclaim liability for indirect, incidental, or consequential damages arising from your use of the Service.
Dispute Resolution
If a dispute arises, you agree first to contact CoverWell to try to resolve it informally. If unresolved, disputes will be governed by binding arbitration under applicable state law. You waive the right to participate in class actions.
Modifications to Terms
We reserve the right to update or change these Terms at any time. Modifications become effective immediately upon posting. Continued use constitutes acceptance of the revised Terms.
